NDHC Invites Survey Participation to Improve Local Health Services

Northwood Deaconess Health Center (NDHC), in collaboration with Grand Forks Public Health, is gathering input from you, the community, to better understand the health needs and priorities of the surrounding communities.


The 2025 Community Health Needs Assessment (CHNA) survey is available online through June 30. Paper copies will also be available at all NDHC clinics or by calling (701) 587-6471.



All residents of Northwood, Larimore, Binford, Aneta, Arvilla, Sharon, and Hatton, as well as surrounding communities , are encouraged to participate.


The CHNA initiative assesses community baselines, increases public health knowledge, strengthens partnerships within our health community, and identifies strengths and weaknesses to address where improvements can be made.


The survey is an integral part of a comprehensive assessment NDHC completes every three years. Past surveys have led to enhanced services including the foot clinic, Meals on Wheels, mental health services in the clinic, and Senior Life Solutions.


Participation is voluntary, and all responses will be kept confidential. Results will be shared in a community need health assessment implementation plan later this year, which will outline how participant feedback will to help guide NDHC forward.
