$50 Sports Physical: includes only what is required to participate in high schools sports. Any additional health concerns will require a separate appointment. Cash payment required at check in.
Preventative Wellness Physical: Must be scheduled in advance and will include a history & exam with all components of a full wellness visit based on the child's age. When scheduling, let us know this is a preventative visit plus sports physical. Cost for this visit will be billed as a preventative physical, based on age and other factors. Call our office for details. Insurance will be billed, with copayments the responsibility of the guarantor. Please check coverage prior to visit.
