Grant award funds transportation support for patients age 65+

NDHC is now offering transportation assistance for patients age 65+ to attend medical appointments.
With a rural health grant from Blue Cross Blue Shield of North Dakota Caring Foundation financial help or coordination for a driver will alleviate transportation barriers and help patients receive medical care.
“Through NDHC’s recent Community Health Needs Assessment survey, transportation was among one of the top barriers identified for aging patients to receiving care. We hope this pilot program can help alleviate this barrier,” says NDHC Quality Manager Ann Gader, R.N., "We are excited to partner with the Caring Foundation to launch this program and enhance access to care for our aging patients."
NDHC will help coordinate a driver to provide rides to or from appointments for those in need. The grant funds will also provide stipends to offset the cost of fuel for transportation. Those using the service must be a patient of NDHC or a Senior Life Solutions client and complete a referral request form.
Medical appointments at NDHC’s Northwood or Larimore clinic or regional appointments in Fargo and Grand Forks will be considered.
