The Nursing Services Department Manager is designated as the person responsible for the implementation of the day by day nursing care policies and procedures.
A sufficient number of RN's, LPN's and Certified Nursing Assistants are on duty 24 hours a day to carry out the policies, responsibilities, and programs of the facility. Care of the resident is their primary concern.
Key professional and other staff members are continually involved in developing and reviewing a plan of care for each resident that is appropriate and individualized to meet their specific needs. The care plan includes problems, approaches, and goals to obtain or maintain each residents highest possible level of functioning.
Family and resident participation in the care planning process is highly encouraged. A care plan conference is held shortly after admission and quarterly thereafter, unless there has been a significant change in the resident's health status. Residents will receive an oral invitation and written reminder of the care plan just prior to the meeting. Family will receive a phone call from Social Services two weeks in advance.
The Medical Director, Dr. Jon Berg, acts as our medical consultant and is primarily responsible for the overall care offered by this facility.
A physician is available 24 hours a day for medical needs and rounds are made on a weekly basis.
Residents in Long Term Care are seen by a physician monthly or more often if needed.
If necessary to call a physician after hours, nursing will contact the physician on-call at the Northwood Deaconess Hospital.
All medications are ordered by the attending physician and are obtained from the pharmacy of the resident's choice. Medications are dispensed by nurses (RN's and LPN's), unless the resident is capable of and wants to administer his/her own.
